In -Home Support Program
The Cook County In-Home Support Program is licensed by the Department of Human Services under statute 245 D to provide services under Cook County Public Health and Human Services. The Cook County In-Home Support Program serves Cook County residents of any age who have a permanent or temporary disability and individuals age 65 and older (regardless of whether or not they suffer from a disability) who are in need of in-home care. For the purposes of the program, a disability is defined as:
- A serious and persistent mental illness (examples include: depression, bipolar disorder, schizophrenia).
- A disability certified by the Social Security Administration.
- Those with rehabilitative needs and a doctor’s order for the service (example: someone who has suffered a recent injury and has been ordered to be non-weight bearing).
Services Include:
- Homemaker/home management (light and heavy housekeeping, laundry, meal preparation, arranging for transportation, organization, etc.)
- Adult companion services
- Respite
- Personal Support
