The Bridge to Health Survey includes population-based health data on adult residents in northeastern Minnesota and northwestern Wisconsin. The goal of the survey is to provide information that will enable organizations and communities to better understand the health of the region’s population.
The Minnesota Student Survey includes questions about a wide variety of youth behaviors, including risk behaviors such as alcohol, tobacco and other drug (ATOD) use, violence and sexual activity, as well as positive behaviors and connection to family, school and community. Survey results are searchable by county.
The U.S. Department of Housing and Urban Development (HUD) developed the Healthy Communities Index (HCI) “to respond to the lack of uniform, evidence-based community health indicators that many cities encountered when they attempted to evaluate the health and well-being of their community.” The HCI is a carefully selected collection of neighborhood-level indicators that measure the social, economic, and environmental conditions necessary for the health of a community.
Using the HCI as a guide, those same indicators have been measured for the neighborhoods of Duluth, MN. The goal of this work is to foster collaboration and understanding based on a common set of facts, help our community identify and prioritize health objectives, and communicate the state of that community to Duluth leaders and residents.
Healthy Northland, a partnership with the seven northeast counties, provides a web-based platform to find recreational maps and healthy outdoor opportunities. Healthy Northland seeks to improve the quality of life and health of all people in northeastern Minnesota and is made possible by a Community Wellness Grant (funded by the Centers for Disease Control and Prevention), and the Statewide Health Improvement Partnership (SHIP) (funded by the Minnesota Department of Health).
Immunization Trends in St. Louis County
Information for immunization rates were pulled from the Minnesota Immunization Information Connection (MIIC) registry, which is run by the Minnesota Department of Health. Data was gathered between June 18, 2019 and September 10, 2019.

PLACES: Local Data for Better Health in St. Louis County
PLACES provides model-based population-level analysis and community estimates to all counties, places (incorporated and census designated places), census tracts, and ZIP Code Tabulation Areas (ZCTAs) across the United States. The data used for PLACES comes from The Behavioral Risk Factor Surveillance System (BRFSS). BRFSS is the nation’s premier system of health-related telephone surveys that collect state data about U.S. residents regarding their health-related risk behaviors, chronic health conditions, and use of preventive services.
