Assertive Community Treatment

Assertive Community Treatment (ACT) is a comprehensive service program that provides community based treatment to adults with serious and persistent mental illness in Carlton County. Assertive Community Treatment may be appropriate when other services such as traditional case management, community support, or adult rehabilitative mental health services are not sufficient to reduce or eliminate debilitating symptoms of mental illness or prevent recurrent episodes of the illness.

  • Budgeting and healthy lifestyle skills
  • Employment skills
  • Household management
  • Interpersonal communication skills
  • Medication education and monitoring
  • Relapse and crisis prevention skills
  • Utilization and integration of community resources
